So often we hear from callers about how frazzled and confused they are with the whole medical industry as they navigate through the process of transferring a loved one from one place to the other. (actually, there is a lot of confusion and anger at the medical industry but that I will save for a book!)
If it makes you feel any better, even those who work within the industry have moments of frustration as they deal with the paperwork, staff, payor sources and red tape involved! Sometimes people have the process completely under control, but their stress comes from siblings or other family members who may not be on board with the decision. Whatever you are facing, believe me, many others are facing it too and you aren’t alone! If you’ve made the decision to move a family member and everyone is in agreement, you’re half way there. The next biggest hurdle is usually deciding where the patient will be moved to and securing that space. (Skilled Nursing Facility/SNF, Assisted Living, Private Home with or without visiting sitters or nursing care). Nursing facilities have people in place to help with the paperwork, the Medicare/Medicaid interstate transfer of paperwork, etc., so utilize them! Be prepared to keep a notebook/journal with everyone you spoke with along with dates,times and key notes.(Especially when dealing with any Government offices)
We will be the easiest part. Really, we will. We hear this time and time again. You work out the paperwork required by the facilities to discharge and admit, and then call us when you have the dates that you want the transfer to take place. As long as the patient medically falls into the criteria of “non emergency”, we will work out the details.
We will maintain medication schedules, continue oxygen if needed, reposition and turn, feed, change and entertain while moving them from point A to point B.
So go ahead and cross all of your “T’s” and dot all of your “I’s” and then call us. At that point, you can relax and either ride along with us or meet us at the destination.
